Protocol Drug doses for Pedi Pt

Pedi dose for ADENOSINE? 0.1 mg/kg (MAX 6mg) with rapid 5-10 ml Saline bolus, 2nd dose 0.2 mg/kg (MAX 12mg)with 20 ml Saline bolus.
Pedi dose for ALBUTEROL? 2.5 mg nebulized at 6 lpm, Same as Adult dose
Pedi dose for ATROPINE? 0.02 - 0.05 mg/kg IV/IO/ET, repeat once to a MAX of 0.04 mg/kg.
Pedi dose for CALCIUM CHLORIDE? 0.2 ml/kg = 20 mg/kg
Pedi dose for DEXTROSE? D25W 2-4 ml/kg if Pt < 34 kg and BSL < 60mh/dl Adult dose if over 34 kg. D25W is made by emptying 25 ml from D50 prefilled syringe and mixing in 25 ml NS
Pedi dose for DIAZEPAM? Broselow Tape may be used or, 0.1 mg/kg IV/IO
Pedi dose for DIPHENHYDRAMINE? same as Adult weight based 0.5 mg/kg MAX 50
Pedi dose for DOPAMINE? Only with Physicans orders. 5 - 20 mcg/kg/min for Neurogenic Shock following volume replacement. Titrate to maintian > 90 SBP.
Pedi dose for EPINEPHRINE? Pulseless / Anaphylaxis 0.01 mg/kg (1:10,000) IV/IO, Moderate Respiratory / Asthma normotensive pt 0.01 mg/kg (1:1000) SQ MAX of 0.3 mg. Additional Epi will need orders from Med Control
Pedi dose for ETOMIDATE? 0.15 - 0.3 mg/kg SIVP over 30 -60 sec
Pedi dose for GLUCAGON? 0.5 mg SQ or IM for pt < 20 kg 1 mg SQ or IM for pt > 20 kg
Pedi dose for LIDOCAINE? 1 mg/kg IV repeat every 10 min to MAX 3 mg/kg. Follow bolus with infusion of 20 - 50 mcg/kg/min. 300 mg into 250 ml NS = 1 micro-drop/kg/min = 20 mcg/kg/min
Pedi dose for MIDAZOLAM? 0.05 mg/kg IV/IO/IM repeat upto 1 time to MAX of 1mg. 0.1 mg/kg IM for seizures repeat once for recurrent seizures
Pedi dose for MORPHINE SULFATE? May only be administered with Physicians orders Burns exceeding 10% BSA (2nd / 3rd) and Long boane Fx 0.1 mg/kg IV/IO MAX of 5 mg
Pedi dose for NALOXONE? 0.1 mg/kg IV, or Nasal if no IV access > 12 yo as needed for resiratory depression. Repeat as needed.
Pedi dose for SUCCINYLCHOLINE? 1.5 mg/kg to 2 mg/kg IVP no repeat doses.
Pedi dose for ZOFRAN? 0.15 mg/kg MAX dose 4 mg IV/IO/IM. 4 mg ODT (oral desintegrating Tablet) if 4 yrs or greater.
