Amiodarone

QuestionAnswer
Type: Potent anti-arrythmic agent
Presentation: Ampoule 150 mg/3ml
Actions: Prolongs action potential duration, inc refratoriness - Blocks Na+ channels - Ca2+ blockade, antiadrenergic effects - Prolongs QT(global prolongation of repolarisation - Delay in AV node conduction
Use: Slow ventricular rate in AF and AFl, to Rx VF & VT
Adv Eff: Vasodilation & neg inotropic effects; hypotension...occasionally brady(elderly), phlebitis, flushes/sweating
Contras: KSAR, cross sensitivity to iodine
Dose: VF: Adult 300mg over 30-60seconds, paed 5mg/kg VT, AF & AFl: 150mg springfuser(10min, 7ml), paed 5mg/kg
Special note: potentiaters: Digoxin, Dilantin, B blockers, Ca channel blockers
