| Question | Answer |
|---|---|
| What is the classification Atropine? | Parasympatholytic |
| What is the pharmacodynamics of Atropine? | Selectively blocks muscarinic receptors inhibiting the parasympathetic nervous system letting the sympathetic take over. |
| What are the indications of Atropine? | - 1st line medication for symptomatic sinus bradycardia - May be beneficial in presence of AV nodal block - Organophosphate poisoning (very large doses required) |
| What are the contraindications of Atropine? | - Allergic to drug - Use with caution in presence of myocardial ischemia & hypoxia causes increased myocardial O2 demand. - Avoid in hypothermic bradycardia - May not be effective for infranodal blocks be prepared to pace |
| What are the Adverse effects of Atropine? | - Blurred vision - Dry mouth - Dilated pupils - Confusion - Doses <0.5 mg may result in paradoxical slowing of the heart |
| What is the dose of Atropine when dealing with bradycardia with or without ACS | - 1 mg IV push every 3-5 minutes as needed, not to exceed total dose of 0.04 mg/kg (total 3 mg) - May use shorter dose interval (3 mins) in severe clinical conditions. |
| What is the dose for Atropine when dealing with organophosphate poisoning? | - 2-4 mg (or higher) IVP |
