what does lidocaine do? depresses conduction in ischemic myocardium
how do we give lidocaine? 1-1.5 mg @ 25-50 mg/min with continuous monitoring or 0.5-0.75mg then reduce by 50% after 24 hrs
what is the 1st line drug for V arrhythmias and used for CPR and A-fib? amiodarone
what are s/sx of amiodarone? hypotension, increased liver enzymes, involuntary movements, prolonged QT, SJS
how do we give amiodarone? 150mg over 10 mins and then infuse @1mg/min
what is the 1st line drug for torsades? mag sulfate
mag sulfate causes what s/sx? hypocalcemia, hyporeflexia, resp. tract paralysis, flushing
what is the dose for torsades and refractory PSVT? torsades- 1-2g over 15 mins PSVT- 3-4g over 30 sec, use extreme caution
what drug is 1st line for bradycardia w/sx? atropine
what is the dose for atropine? 0.5 mg q3-5 mins for max of 3 dose
what is the classification of atropine and its s/sx? anticholinergic can't see,pee, spit, or shit
what drug is used for cardiac arrest, hypotension and bradycardia? dopamine
what parameters do we use for dopamine dosage? <2= renal damage >10= vasodilation
what does dopamine do? increases HR/BP
s/sx to warn when using dopamine? chest pain, HTN, tachycardia, HA, widen QRS complex
what is the dosage for dopamine? 2-10 mcg IV and titrate for responsiveness
what drug slows impulse formation in SA node and conduction in AV node? adenosine
what is the 1st line drug for SVT and diagnosis of tachyarrhythmias? adenosine
what do we monitor after giving adenosine? dyspnea, cardiac arrest, complete AV block, bronchospasms in asthmatics
how do give adenosine Dr present, 6 mg rapid> 20 ml flush failed attempt= 12 mg x2 w/ 20ml flush
what tachyarrhythmic is used for atrial arrhythmias, PSVT, and stable A-fib cardizem
what do we monitor for after given cardizem? bradycardia, peripheral edema, cough, heart block, liver toxicity
what electrolyte raises blood pH? sodium bicarb
we use sodium bicarb for what? cardiac arrest w/ or w/out hyperkalemia (asystole)
what do not want to happen with sodium bicarb? cellulitis, tissue necrosis, metabolic alkalosis
what drug is used for hyper/hypo-calcemia calcium chloride
what does calcium chloride do for the body? causes muscle contractions
what should warn pts of with calcium chloride? burning sensation and vasodilation
what vasopressor is used for hypotensive state after resuscitation? norepinephrine- levophed
what is the solution and amount we mix with for norepi-levophed? 4 mg in 250ml D5W or NS
how do we give norepinephrine-levophed? initial dose= 8-12 mcg/min then maintenance IVof 2-4mcg and titrate
