Conclusion: Written as a paragraph/summary of your findings; using:
Claim: hypothesis was or was not supported and what happened in your experiment
Evidence: specific data from experiment that supports what happened in your experiment
Interpretation: example of how this topic relates to something in your life OR identify a new problem you could investigate
