ADPIE Assessment/Analysis, Data collection, Planning, Implementation, Evaluate
Assessment Collecting data on the patient to find current needs and the necessary precautions to take for the data. Includes patient interview and interviewing the family, initial assessment, focused assessment, ongoing assessments.
Initial Assessments Also called Baseline assessment, includes obtaining initial vitals and conducting the patient interview.
Focused Assessment Takes place after the baseline assessment. Focuses on the main issue the patient presents.
Ongoing assessment The assessing of the concern after the implementation has occurred and continuous monitoring.
What 7 components/skills are involved in the Assessment step of ADPIE? Asking the appropriate questions, listening carefully, head to toe assessment skills, clinical judgement skills, critical thinking skills, collecting assessment data prior to implementation.
What is subjective data? Subjective data is the information the patient tells me about their concern or their health. Immeasurable. Things I cannot detest or say that did not happen. Things I cannot feel for the patient.
Documentation of subjective data should always be in ________. The patients own words.
What is objective data? Objective data is the collected data that tells the healthcare team what is wrong with proof. Lab test results, imaging, assessments the team conducts themselves.
Describe the Analysis step in ADPIE: The analysis part of ADPIE is about interpreting the collected data. Recognizing patterns or trends associated, comparing the data to normal ranges, is focused.
How do you document analytical data? Making sure the data is focused on the fact and is highly descriptive.
