The incident report is intended for use by the healthcare facility’s staff and management to investigate and prevent future incidents.
When is an incident report required and by whom? whenever an accident, injury, error, or unusual event occurs, and it must be completed by the staff member who witnessed or discovered the incident.
Why is the incident report important it helps identify causes of accidents or errors, ensures proper follow-up, improves resident safety, and protects the facility legally.
Who is involved in filling out an incident report? the staff member who witnessed or discovered the incident is responsible for filling out the incident report.
Know the guidelines for completing an incident report Report the incident immediately, Record only the facts without opinions or blame. Include the date, time, place, people involved, and what happened. Describe any actions taken and the resident’s condition. Keep the report confidential.
Remember that HIPPA information such as DOB and SSN are not included in an incident report confidential information like a resident’s DOB, Social Security Number, or other personal identifiers should NOT be included to protect privacy and comply with HIPAA regulations. The report should focus only on the facts of the incident
