Nurse Aide I Module M The Interdisciplinary Care Plan Definition List

Activities of daily living (ADLs) are basic self-care tasks that residents need help with, such as bathing, dressing, eating, toileting, and mobility.
Assessment is the process of gathering information about a resident’s physical, mental, and emotional health to plan appropriate care.
Delegation is the process where a nurse assigns tasks to a nurse aide that are within their scope of practice while remaining responsible for the outcome.
Delegatee is the person (like a nurse aide) who receives a task from a nurse to perform.
Evaluation process of determining if a resident’s goals and care plans are being met and if any changes are needed.
Evidence-Based Practice is using current, research-backed methods and best practices to provide safe and effective care to residents.
Implementation is the act of putting the care plan into action by performing the assigned tasks and providing care to the resident.
Interdisciplinary Care Plan a coordinated plan developed by the whole healthcare team, including nurses, aides, doctors, and therapists, to meet a resident’s individual needs.
Maslow's Hierarchy of Needs a theory that arranges human needs in a pyramid, starting with basic physical needs like food and safety at the bottom, and moving up to psychological needs and self-actualization at the top.
Nursing Diagnosis a clinical judgment about a resident’s health problems that guides the nurse aide and healthcare team in planning care
Physiological refers to the functions and processes of the body and how its systems work to maintain health and life.
Planning is the step in the nursing process where goals are set and specific actions are decided to meet a resident’s needs.
Self-Actualization is the highest level in Maslow’s hierarchy, where a person achieves their full potential and personal growth.
Self-esteem a person’s sense of self-worth and confidence in their own abilities and value.
Therapeutic refers to actions or communication that promote healing, comfort, and well-being for the resident.
Unlicensed Assistive Personnel (UAP) are healthcare workers, like nurse aides, who provide basic care under the supervision of licensed nurses but do not have a nursing license themselves.
