Nurse Aide I Module L Communicating with the Health Care Team Definiton List

TermDefinition
Communication with the Health Care Team involves clearly and accurately sharing information about residents’ care, condition changes, and concerns with nurses, doctors, and other team members to ensure safe and effective care.
HIPPA is a federal law that protects the privacy and security of a person’s health information and ensures confidential handling of medical records.
Medical Record a legal document that contains all the detailed information about a resident’s health history, care, treatments, and progress.
Objective Data is information that can be observed, measured, or verified by the nurse aide, such as vital signs, skin color, or swelling.
Recording is the process of accurately documenting all care given and observations made about a resident in their medical record.
Reporting is the act of promptly informing the nurse or healthcare team about any changes in a resident’s condition, concerns, or incidents that require attention.
Subjective Data is information reported by the resident about how they feel, such as pain, nausea, or dizziness, which cannot be directly observed or measured.
