Pharmacy Practice Act

Pharmacy - Definition Every store, shop, pharmacy department, or other place where pharmacist care is provided by a pharmacist
Practice of Pharmacy - Definition Interpretation and provision of assistance in the monitoring, eval, & implementation of Rx drug orders; dispensing of Rx drug orders, patient education & proper use or delivery of meds, participation in drug & device selection, immunization of those > 7
Required Components of a Prescription Name of Patient Date issued Name & strength of drug or description of medical device Quantity Directions for use Prescriber's name, address, and signature; and DEA registration number (controlled) Noncontrolled are valid up to 15 months from date
Compounding - Definition The preparation and mixing of components, excluding flavorings. Commercially available products may be compounded if: product is not reasonably available in a timely manner to meet the patient's needs & the prescriber has requested the compounding
Tech Role in Patient Counseling A pharmacy tech may only participate by: obtaining a medication history, providing the offer for counseling by a pharmacist/student pharmacist, and acquiring a patient's allergies and health conditions
Pharmacy Technician Guidelines Must be 16 years or older May be delegated to perform any task they're trained for, except patient counseling, drug regimen review, clinical conflict resolution, or final prescription verification
Supportive Staff Those who solely perform clerical work are not required to be licensed as a registered pharmacy technician. This includes operating registers, sorting pre-packaged drugs in centralized setting, and selling prescriptions already final verified.
Certified Pharmacy Technician Guidelines Attained the age of 18, passed an examination accredited by NCCA, paid the required fees, and completed 20 hours of CE during the 24 months preceding license expiration (1hr in law, 1 hr in patient safety)
Board of Pharmacy Consists of 11 members 8 are licensed pharmacists (one with inpatient hospital pharmacy)(all with 5 years practical experience) 1 is a pharmacy technician 2 are public members All serve five year terms
Duties of Department Appoint a chief pharmacy coordinator (licensed RPh, >5 years experience) Appoint pharmacy investigators who report to above (also a licensed RPh; have the right to enter and inspect) Employ one attorney to prosecute violations of Act
License Expiration, Renewal Licensee must complete a total of 30 hours of pharmacy CE during the 24 months preceding expiration
Shift Requirements A pharmacist, student pharmacist, or technician shall not work longer than 12 continuous hours per day. For each 6 continuous hours or longer, they are allowed, at minimum, one 30-minute uninterrupted meal break and one 15-minute break.
Emergency Refills 1: per RPh, gap in therapy may cause harm 2: Previously dispensed or refilled for same pt/medication 3: Med is not controlled 4: informs pt pres. auth. is needed for future refills 5: Refill is documented & prescriber notified 6: NTE 30-d supply
Cessation of Pharmacy Operations The PIC shall notify the department and forward a copy of the closing inventory of controlled substances and a statement indicating the intended manner of disposition of all legend drugs and prescription files within 30 days
Technician Training Following Areas: duties/responsibilities of staff, tasks & technical skills/procedures/policies, compounding/packaging/labeling/storage, terminology, record keeping requirements, arithmetic calculations WITHIN 2 yrs of initial licensure
Record Retention Suitable book, file, or electronic record of the original Rx, or unalterable image for not less than 5 years
Transferring If (re)filling a valid prescription on file in another state, RPh must follow all requirements of IL law C3-5: may be transferred only once Pharmacies sharing real-time, electronic database may transfer up to maximum refills
Opioid Antagonists A licensed pharmacist shall dispense an opioid antagonist in accordance with written, standardized procedures/protocols Before dispensing an opioid, RPh shall inform pt that opioids are addictive and offer an opioid antagonist
Biological Substitution An RPh may substitute if the product is approved as interchangeable, the prescriber allows substitution, and the pharmacy informs the patient. RPh should contact the prescriber electronically within 5d of substitution, must include name of product
Do Not Substitute Requirements The prescriber shall sign the form in their own handwriting to authorize the issuance of the prescription. Preprinted or rubber stamped marks, or other deviations are not permitted
Electronic Visual Image Prescriptions If the computer system can capture an unalterable image of the Rx drug order, thee electronic image shall constitute the original prescription and a hard copy is not required. image should be providable within 72 hours of request
Dispensing Error - Definition Any preventable event that may cause or lead to inappropriate medication use or patient harm
Patient Counseling - definition Communication between pharmacist/student and a patient/agent about the patient's medication or device for the purpose of optimizing proper use of prescription medications or devices
Immunization Recordkeeping/Reporting All records regarding each administration of a vaccine must be kept for 5 years. A pharmacist must ensure that the report of that administration, is made to (I-CARE) or to the primary healthcare provider within 30 days of administration
Closing a Pharmacy Provide notice to Drug Compliance Unit no later than 20 days after Notify customers 15 days in advance and where their records will be maintained Conduct an inventory, keep for five years Cover all signage ASAP. Notify division where controlleds go
