112-T2 Inhalers - SMI, MDI, DPI

QuestionAnswer
MDI (Metered Dose Inhaler) - deliver drugs as a fine liquid spray - use a propellant to form an aerosol - can be used with or without spacer (spacer IS preferred)
DPI (Dry powder inhaler) - deliver drugs as a fine powder - rely on patient's inhalation to pull and disperse drugs into lungs - do NOT use propellants or spacers
SMI (Soft Mist Inhaler) (also called Fine Mist Inhalers) - create an aerosol or mist - use a mechanical spring-loaded device - do not require propellants or spacers - particles are smaller, allowing more drug to reach lower airways
MDIs require coordination of activation and inhalation
DPIs are breath activated, requiring less coordination
SMIs deliver a slower moving mist, potentially easier to inhale
to reduce oral deposition and improve drug deliver, MDI's can use spacers
Do not use spacers: DPIs and SMIs
which inhalers typically require priming MDIs shake inhaler, release one or more sprays away from face (repeating as directed by manufacturer)
When should an MDI be primed? new MDIs before 1st use re-prime after extended periods of non-use (often 1-2 weeks)
DPI's and SMIs generally do NOT require priming
Breath Holding MDI - 5-10 sec DPI - 5-10 sec SMI - 10 sec SMIs have a slower moving mist that take longer to fully deposit in lungs and the fine particles can penetrate deeper and remain suspended in air longer and extra time ensures medication reached intended areas of lungs
canisters of MDIs (metered dose inhalers) should be shaken for -5-10 seconds between each inhalation - do not shake DPIs because it could disrupt the powder dose - SMIs typically do not require shaking (follow product instruction)
Exhale fully before using any inhaler (DPI, MDI, or SMI) to create more space for medication
inhale slowly and deeply with MDIs and SMIs
inhale quickly and forcefully with DPIs
for multiple doses of the same medication typically wait 30-60 seconds between puffs of the same medication. This allows time for first dose to start working and creates space for next dose
for combination doses wait - 1-2 minutes between different medications - always use a bronchodilator first if combining with a corticosteroid - for rescue inhaler follow manufactures instructions
Slow pursed-lip exhalation is recommended after using any inhaler
nebulizers - convert liquid medication into a fine mist - deliver medication thru a mask or mouthpiece (or trach) - take longer than inhalers (10-15 min) - are often used for larger doses - benefit those who have difficulty with inhalers
Corticosteroids are typically delivered with MDIs and DPIs. After inhaling a corticosteroid, it is important to - rinse thoroughly with water, gargle softly, and spit - rinse after 20-30 minutes

Mouth placement for Dry Powder Inhalers (DPIs): Place mouthpiece between teeth Close lips tightly around mouthpiece Do not block air vents with fingers
Mouth placement for Soft Mist Inhalers (SMIs): Hold inhaler horizontally Place mouthpiece between teeth Close lips tightly around mouthpiece
Mouth placement for Spacer or Holding Chamber (with MDI): Place mouthpiece of spacer between teeth Close lips tightly around mouthpiece
DPI is short for dry powder inhaler
SMI is short for soft mist inhaler (also fine mist inhaler)
MDI is short for metered dose inhaler
