Introduction terms
| Term | Definition |
|---|---|
| Legibility | The order cannot be read due to handwriting |
| Prescriptive | The drug selection is incorrect based on the clients\'s allergies or other indications such as the wrong dose, form, quanity, route, and other things |
| Communication | a lack of understanding the correct order; confusion over look alike and sound alike medications |
| Transcription | The order was transferred onto another source incorrectly |
| Preparation | the wrong dilution or solution was used |
| Administration | given at the wrong time via incorrect route |
| Wrong patient | the medication was not given to the client in which it was intended |
| Distraction | something else required the prescriber, transcriber, or nurse's attention |
| Fatigue | lack of sleep on behalf of the prescriber or nurse |
| Lack of knowledge/understanding | the prescriber and/or the nurse was not aware of how the drug works, its various names, the side effects or contraindications or unsure of how to adminster the medication; that also includes using the wrong equipment |
| Incomplete Client-Information | due to incomplete reconciliation of medications, health problem or separation of clients with the same names, could also include the wrong client weight |
| Memory Lapses | forgetting some information such as allergies or failure to administer |
| Systematic Issues | Improper storage, preparation, and availability |
