skin integrity and wound care

| dressing | protective covering place over the wound. |
| epidermis | top layer or outermost portion of the skin |
| eschar | thick leathery scab or dry crust that is necrotic and must be removed before the stage is determined. |
| evisceration | wound completely seperated and protrusion of viscera through the incisional area. |
| exudate | fluid that accumulates in the wound, may contain serum,cellular debris,bacteria and white blood cells. |
| fistula | abnormal passage from an internal organ ot the outside of the body. |
| granulation tissue | new tissue that is pink or red in color and composed of fibroblasts and small blood vessels that fill an open wound when it starts to heal. |
| ischemia | poor circulation in particular area |
| maceration | over dehydration |
| necrosis | death of tissue |
| pressure ulcer | wound with a localized area of tissue necrosis |
| scar | avascular collagen tissue that doesnt sweat,grow hair, or tan in sunlight and eventually becomes a flat,thin line. |
| shearing force | when on layer of tissue slide over anothr layer |
| wound | break or disruption in the normal integrity of the skin and tissues. |
