skin integrity and wound care

bandage strips of cloth,gauze,or elasticized material used to wrap a body part.
debridement cleaning away devitalized tissue and foreign material.
dehiscence partial or total seperation of wound layers as a result of excessive stress on wounds that are not healed.
dermis framework of elastic connective tissue
dessication dehydration
dressing protective covering place over the wound.
epidermis top layer or outermost portion of the skin
eschar thick leathery scab or dry crust that is necrotic and must be removed before the stage is determined.
evisceration wound completely seperated and protrusion of viscera through the incisional area.
exudate fluid that accumulates in the wound, may contain serum,cellular debris,bacteria and white blood cells.
fistula abnormal passage from an internal organ ot the outside of the body.
granulation tissue new tissue that is pink or red in color and composed of fibroblasts and small blood vessels that fill an open wound when it starts to heal.
ischemia poor circulation in particular area
maceration over dehydration
necrosis death of tissue
pressure ulcer wound with a localized area of tissue necrosis
scar avascular collagen tissue that doesnt sweat,grow hair, or tan in sunlight and eventually becomes a flat,thin line.
shearing force when on layer of tissue slide over anothr layer
wound break or disruption in the normal integrity of the skin and tissues.
