What is the epidemiology of salmonella? - intestinal organism found in the GI tract -some are host-restricted (S.typhi) vs host-adapted (S. Choleraesuis) -grows between 8 and 45c -Rapid generation time -Susceptible to freezing/drying
What is the mode of transmission for salmonella? -fecal contamination: food, water(week), soil (years) -Person to person -Travel -reptiles
What diseases does salmonella cause? -gastroenteritis -enteric fever aka typhoid fever -Septicemia -carrier state
gastroenteritis (symptoms) -diarrhea: watery to cholera-like -fever -abdominal pain -nausea -vomiting -malaise
Typhoid fever- week 1 untreated - blood, urine, blood marrow= negative -stool or transiently positive -diarrhea -malaise -fever -abdominal pain -muscle aches -headache -anorexia
Typhoid fever- week 2 untreated -established disease -positive: blood, stool, bone marrow, urine, rose spots -bacteremia -leucocytosis -hepatosplenomegaly -rose spots -neurological disturbances
Typhoid fever- week 3-5 untreated -late stage, convalescence -blood= negative(unless relapse), stool is variable, possible for urine -relapse -intestinal hemorrhage/perforation -osteomyelitis -focal infection
Septicemia or focalized infection - 3 to 16% -Patients immunocompromised or have underlying disease -90% have GI sx prior or during advanced disease -Focalized infection * UTI, cholecystitis, osteomyelitis, meningitis, peritonitis
What media should be used to distinguish pathogenicity for salmonella? -media: TSI, LIA, and MIO -urea, spot indole -conventional bc
S enterica is comprised of 5 subspecies I enterica II salamae IIIA arizonae IIIB diarizonae IV houtenae VI indica
