Dysrhythmias/CA
| Question | Answer |
|---|---|
| A-fib | No P waves or PR interval, atria quivering |
| A-fib medication | Amiodarone, metoprolol, digoxin, heparin |
| A-fib, A-flutter treatment | Medication, cardioversion, catheter (radiofrequency) ablation, maze procedure with cryoablation |
| Cardioversion | Shock to reset heart to normal rhythm, synchronized to shock on R. Maintain airway (sedated). Informed consent |
| Catheter (Radiofrequency) ablation | Heat destroys or shrinks abnormal tissue |
| Maze procedure with cryoablation | Cold energy to create scar tissue pattern in heart |
| Amiodarone | Treats and prevent arrythmias Monitor BP, ECG, signs of heart failure |
| Second degree block, type 1 (Mobitz 1/Wenckebach) | "If PR gets longer, then a QRS dop" Can be due to ischemia, drugs |
| Wenckebach treatment | Symptomatic = Atropine + pacemaker Asymptomatic = Monitor |
| Second degree block, type 2 (Mobitz 2) | "If PR stays normal, and QRS quits" Can be due to heart disease, drug toxicity |
| Mobitz 2 treatment | Pacemaker |
| Third degree block | Normal P wave, no connection with QRS complex Low heart rate <40bpm |
| Third degree block treatment | Transcutaneous pacing or permanent pacemaker. Dopamine and epinephrine to increase HR while waiting for procedure |
| Premature ventricular contraction/depolarization | Can cause more serious dysrhythmias, assess hemodynamic status, no shower for 1-2 days, Holter Monitor |
| Holter Monitor | Portable ECG, to measure over a period of time |
| V-Tach | Looks like a saw, patient is hypotensive Stable = pulse Unstable = pulseless |
| V-Tach treatment | Stable = amiodarone, cardioversion Unstable = CPR, defibrillation |
| V- Tach Torsades de Pointes | Polymorphic, treatment is needed or can turn into V-fib |
| Torsades de Pointes treatment | Magnesium Sulfate |
| V-Fib | Patient is unconscious, if not treated rapidly death will result |
| V-Fib treatment | D-fib, CPR, amiodarone 300mg push |
| Asystole (flat line) treatment | CPR, epinephrine, intubation |
| ICD (implantable cardioverter defibrillator) | Monitors the heart rhythm and delivers an electrical shock if it detects a life-threatening arrhythmia |
| Pacemaker | Monitors the heart rhythm and sends electrical pulses to regulate it when it beats too slowly |
