Drug administration: all routes and necessary instructions

nasal administration have client blow nose prior, lay supine with head tilted back, remain supine and avoid blowing nose for a few minutes
ophthalmic administration tilt head back, have client look up, place hands on client's forehead, pull skin beneath eye down, instill eyedrops to lower outer conjunctival sac, close eye and apply pressure to inner canthus
teaching ophthalmic administration to client do not allow tip to touch eye, close eyes and hold pressure for 30-60 seconds (prevents systemic absorption)
otic drug administration side-lying position for client, place hands against head and above pinna, straighten ear canal, press tragus 2x, place cotton into meatus of external canal
otic administration children under 3 pull pinna down and back because ear canal is straight and directed upward
otic administration adults pull pinna up and and back because canal is "S" shaped
insertion location of PICC basilic or cephalic vein one finger above or below antecubital fossa in lower 1/3 of superior vena cava
implanted port management must flush after every use and once a month
insertion location of implanted port subclavian vein to superior vena cava, left completely under the skin
tunneled percutaneous central catheter only end of catheter is exposed outside of skin
insertion location nontunneled percutaneous central catheter subclavian or jugular vein into tip of of distal third vena cava
how much residual indicates medication should be held more than 100 mL
what size syringe is used for flushing tubes 60 mL
tube administration place towel under tube, do not mix medications, dissolve medications with water, flush 15-30mL warm water or sterile water before and after administration of each drug
5 rights of medication administration right patient, right medication, right time, right route, right dosage
topical medication administration assess area for redness, swelling, irritation, and drainage
what should be documented for vaccine administration date, route and site, type manufacturer, lot number, expiration date, name, address, and signature
