Angiotensin II Receptor Antagonists Antihypertensives
Indications HTN, CHF for pts who can't tolerate ACE inhibitors
Action Block vasoconstrictor and aldosterone porducing effects of angiotensin II at receptor sites, including vascular smooth muscle and the adrenal glands
Therapeutic Effect Lowering of blood pressure, slowed progression of diabetic nephropathy, decreased risk of stroke
Ends in -sartan
Examples Losartan, valsartan, oprosartan
Contraindications Pregnancy/lactation
Use cautiously CHF, Impaired renal function, black pts, may not work
Nursing Assess BP, angioedema, in CHF intake, outpput, breath sounds
Labs Creatinine, bilirubin, potassium
Angioedema Dyspnea, facial swelling
Patient teaching Change positions slowly, may cause dizziness,
Adverse effects dizziness, fatigue, HA, hypotension, diarrhea, hyperkalemia
