Calcium Channel Blockers HTN, Angina, supraventricular tachyarrhythmias, atrial flutter, fibrillation
Action Inhibits transport of calcium into myocardial and vascular smooth cell, resulting in inhibition of excitation-contraction coupling and subsequent contraction
Therapeutic effect Systmeic vasodilation resulting in decreased blood pressure, suppression of angina
Contraindications 2nd or 3rd degree heart block, blood pressure less than 90, recent MI, pulmonary congestion, concurrent use of rifampin
Adverse reactions periperal edema, gingival hyperplasia
Grapefruit juice increases serum levels and effect
Nursing Assess for signs of CHF
Labs Potassium, hypokalemia increases risk of arrhythmias and should be corrected, may cause an increase in hepatic enzymes
