Pernicious Anemia

What is pernicious anemia | Elderly effected mostly,Vit. B def. which is needed for DNA synthesis in maturing RBC. Autoimmune gastric mucosal atrophy. B12 is not absorbed. |
Drugs that interfere with absoption of b12 | ascorbic acid (vit. C), colchocine, neomycin, oral contraceptives |
Signs and Symptoms | low B12 levels,Pallor, fatique, palpitations, Angina, CHF, inflammation of tongue and mouth, sore mouth, anorexia, nausea, gi upset, jaundice, neurological issues like numbness in hands and feet, gait off and BB dysfunction |
Diagnostic and Labs | CBC, Blood smear, rbc, wbc, hematacrit, platelet count, gastric analysis and Schilling Test |
What is Schillings Test? | Small amount of radioactive B12 orally given and then 24 hr urine collecton to measure uptake of B12 to see if b12 is being absorbed. |
Treatment of Pernicious Anemia | IM B12 injections, quiet environment and reorietate pt if needed, safe environment, physical or occupational therapy may be needed. |
Pt teaching | advise to have monthy B12 injection for life, see doctor approx. every 6 months for hematologic sudies and GI evaluation, stool exam for accult blood, gastric cytology, and thyroid function test. |
