Step 1: Inspect Anterior Chest Apical pulsations, heaves, bulges.
Step 2: Palpate Anterior Chest Apical pulsation, thrills, lifts heaves, bulges.
Step 3: Auscultate Apical Pulse Rate, rhythm.
Step 4: Auscultate Heart Sounds with Bell and Diaphragm Clarity, extra sounds. Most defined S1 and S2.
Step 5: Inspect and Palpate Skin of Upper Extremities Symmetry, edema, skin integrity, color temperature.
Step 6: Inspect Fingernails Hygiene, texture, strength, intactness of cuticles, color of nail beds, nail base angle, and capillary refill.
Step 7: Palpate Upper Extremity Pulses Carotid, brachial, and radial. Strength, rhythm, equality bilaterally.
Step 8: Inspect and Palpate Skin of Lower Extremities Symmetry, edema, skin integrity, color temperature.
Step 9: Inspect Toenails Hygiene, texture, strength, intactness of cuticles, color of nail beds, nail base angle, and capillary refill.
Step 10: Palpate Lower Extremity Pulses Femoral, popliteal, posterior tibial, and dorsalis pedis. Strength rhythm, equality bilaterally.
Step 11: Inspect Skin for Lesions Suspiciousness. Typre,symmetry, border, color, diameter, elevation.
Steps: 1:Inspect Anterior Chest, 2:Palpate Anterior Chest, 3:Auscultate Apical Pulse, 4:Auscultate Heart Sounds, 5: Inspect Skin of UEs, 6:Fingernails, 7:Palpate UE Pulses, 8:Inspect Skin of LE, 9:Inspect Toenails, 10:Palpate LE Pulses, 11:Lesions
