Amiodarone VF/pulseless VT: 300mg IVP; 150mg after 3-5 minutes if no response. VT with pulses: 150mg over 10 minutes
Aspirin 162-324mg, chewed
Atenolol 5mg for 5 minutes; repeat dose in 10 minutes
Atropine 0.5mg-1mg IVP; repeatable to a max. of 3mg
Calcium Chloride 1g, repeated PRN.
Calcium Gluconate 1g, repeated PRN.
Dexamethasone 1mg/kg IV bolus
Dextrose (D50) 12.5-25g IVP
Diazepam 5-10mg slow IVP
Diltiazem 0.25mg/kg over 2 minutes
Diphenhydramine 25-50mg IM, IVP
Dopamine Dilute 200mg in 250mL D5W; 2-20mcg/kg/min, titrated to effect
Epinephrine 1:1,000 0.3mg IM or SQ
Epinephrine 1:10,000 1mg IVP q 3-5 minutes during arrest; double dosage for admin. via ETT
Etomidate 0.2-0.6mg/kg IV infusion over 30-60 seconds
Fentanyl 1mcg/kg slow IVP
Furosemide 0.5-1mg/kg IV over 1-2 minutes
Glucagon Hypoglycemia: 1mg IM Beta-blocker OD: 3-10mg IV over 10 min.
Haloperidol 2-5mg q 30-60 min.
Ipratropium 0.5mg/mL for neb.
Labetalol 10mg IVP
Lidocaine Initial: 1.5mg/kg; repeat dosage of 0.75mg/kg q 10 min. To a max of 3mg/kg total.
Lorazepam 2-4mg IM or IV; may be repeated
Magnesium Sulfate Seizures: 1-4g over 3 minutes. Torsades: 1-2g for 5 minutes. Asthma: 1-2g over 3 minutes.
Metoprolol 5mg dose q 5min x 3 rounds.
Midazolam 2-5mg IVP
Morphine Sulfate 2-4mg IV, repeatable
Naloxone 0.4-2mg IV, IM, IN, SQ, ETT
Nitroglycerin Angina: 0.4-1.2mg SL CHF: 0.4-1.2mg 0.5-1 inch paste
Ondansetron 4mg IV, IM; repeatable
Oral Glucose 15-45g PO
Oxygen 1-15 LPM
Rocuronium 0.1mg/kg slow IVP
Sodium Bicarbonate 1mEq/kg IVP
