Assisting with ambulation/fall

#1 Verify orders. (Physicians order if skill is dependent. Independent skills are verified with nursing care plan, Kardex or Charge nurse.Gather equipment. (wheelchair/chair, nonskid shoes for client, gait belt.
#2 *Introduce self to patient. *Wash hands*Identify patient by 2 methods.(check arm band, have patient state name) *Explain procedure*Provide privacy for client
#3 Assist client with puttin on nonskid shoes and bathrobe. *Apply gait belt around the waist. Assist client to stand at bedside.Call for extra help if needed.
#4 Take a few steps. (walk on clients weakest side)Grasping gait belt at center of back and on side. Use assistive device if needed.(walker, cane)
#5 Return client to room and assist back into bed.Remove gait belt and shoes. Raise side rails and leave bed in lowest position.
#6 Be certain client has a way to call for help and knows how to use it. Place needed items in reach.
#7 Wash hands. DOCUMENT! date,time and signature. Distance ambulated.Number of assistants required. Assistive devices used. Client tolerance.
