Nasogastric Tube
PNS2 Inserting tube

| Step 1 | Have suction available. |
| Step 2 | Place patient in high fowler's position. |
| Step 3 | Assess nasal passages with penlight. |
| Step 4 | Assess gag response by gently placing the tongue blade against uvula. |
| Step 5 | Explain how patient can communicate during procedure. |
| Step 6 | Measure tube for length of insertion and mark length with tape or permanent marker. |
| Step 7 | Cover patient's chest with towel. |
| Step 8 | Have patient hold emesis basin and cup of water. |
| Step 9 | Coat tip of tube with water-based lubricant. |
| Step 10 | Slowly insert tube into naris. |
| Step 11 | Instruct patient to touch chin to chest. |
| Step 12 | Advance tube each time patient swallows until reaching marked spot. |
| Step 13 | For resistance, significant coughing, gagging, choking, or changes in vital signs, pull back on tube until resolved. |
| Step 14 | Use penlight to look at back of throat. |
| Step 15 | Auscultate for breath sounds. |
| Step 16 | Using syringe, withdraw gastric contents, assess aspirate, and test pH. |
| Step 17 | Anchor tube to nose. |
| Step 18 | Mark tube directly below naris. |
| Step 19 | Fasten tube to gown. |
| Step 20 | Keep head of bed above 30 degrees. |
