Primary Lesions

When lesion develops on previously unaltered skin

Macule Solely a color change, flat and circumscribed of less than 1 cm freckles, flat nevi, hypopigmentation, scarlet fever
Patch Macules that are larger than 1 cm monogolian spot, vitiligo, measles rash
Papule Something you can feel less than 1 cm diameter, due to superficial thickening in epidermis elevated nevus (mole), lichen planus , wart (verruca)
Plaque Papules coalesce to form surface elevation wider than 1 cm. psoriasis, lichen planus
Nodule Solid, elevated, hard or soft , larger than 1 cm. May extend deeper into dermis than papule Xanthoma, fibroma, intradermal nevi.
Tumor Larger than a few centimeters in diameter, firm or soft, deeper into dermis; may be benign or malignant. Lipoma, Hemangioma
Wheal Superficial, raised, transient, and erythmatous; slightly irregular shape due to edema Mosquito bite, allergic reaction, dermographism
Urticaria (Hives) Wheals coalece to form extensive reaction, intensely pruritic (blank)
Cyst Encapsulated fluid-filled cavity in dermis or subcutaneous layer, tensely elevating skin Sebaceous cyst, wen
Vesicles Elevated cavity containing free fluid, up to 1 cm. Clear serum flows if wall is ruptured herpes simplez, early varicella (chickenpox), Shingles
Bulla Larger than 1 cm diameter, Unilocular, Superficial in epidermis, it is thin walled and ruptures easily friction blister, pemphigus, burns
Pustule Turbid fluid (pus) in the cavity. Cirrcumscribed and elevated Impetigo, acne
