| Skin Integrity Delegation | Assessing may not be delegated; NAP may reposition pt's position and specific positions individuated for the pt, report any redness or breakage, report any abrasions from medical devices |
| Dermal-epidermal junction | a membrane that separates the epidermis and the dermis |
| Basal layer | the innermost epidermal layer |
| Collagen | a tough fibrous protein |
| Pressure ulcer | Impaired skin integrity related to unrelieved, prolonged pressure |
| Tissue ischemia | when the pressure applied over a capillary exceeds the normal capillary pressure and the vessel is occluded for a prolonged period of time |
| slough | stringy substance attached to the wound bed |
| eschar | black, brown, tan or necrotic tissue |
| Granulation tissue | red, moist tissue composed of new blood vessels, the presence of this indicates healing. |
| Friction | the force of two surfaces moving across one another such as the mechanical force exerted when skin is dragged across a coarse surface such as bed linens |
| Exudate | fluid, cells, or other substances that have been discharged from cells or blood vessels slowly through small pores or breaks in cell membranes |
| Wound | a disruption of the integrity and function of tissues in the body |
