| Term | Definition |

| Acute Illness | A sudden illness from which a person is expected to recover. |
| Accreditation for a Health Care Agency | This is a voluntary and rigorous process of being certified as a health care agency/facility that provides excellent quality health care to their patient’s/resident’s |
| Assisted Living Residence | Provides housing, personal care, support services, health care, and social activities in a home-like setting to persons needing help with daily activities |
| Case Management | A nursing case manager coordinates the care of specific groups patients from admission through discharge and into the home or long term care facility |
| Certification for a Health Care Agency | This is a requirement for licensed agencies to receive Medicare and Medicaid reimbursement funds for provision of care. |
| Chronic Illness | An on-going illness that is slow or gradual in onset; it has no known cure; it can be controlled and complications prevented with proper treatment. |
| Health Team | The many health care workers whose skills and knowledge focus on the person’s total care; interdisciplinary health care team |
| Hospice | A health care agency or program for persons who are dying |
| Licensed Practical Nurse (LPN) | A nurse who has completed a practical nursing program and has passed a licensing test |
| Licensure for a Health Care Agency | An agency must have a state license to operate and provide care |
| Nursing Assistant | A person who has passed a nursing assistant training and competency evaluation program; performs delegated nursing tasks under the supervision of a licensed nurse; has passed a state certification competency exam |
| Nursing Team | Those who provide nursing care –RN’s, LPN’s /LVN’s, and Certified Nursing Assistants (CNA’s) |
| Patient-Focused Care | A nursing care pattern; services are moved from departments to the bedside. 2, 3, or 4-year nursing program and has passed a licensing test |
| Surveyor | A person/team from a regulatory agency who collects information by observing and asking questions |
| Terminal Illness | An illness or injury from which the person will not likely recover |
