Nitroglycerin 0.4mg sublingual every 3-5 minutes if pain persists and systolic BP >100
Morphine (STEMI) First dose: 2-4mg IV Additional doses: 2-8mg at 5-15 minute intervals
Metoprolol Class II Beta Blocker
Calcium Chloride 10%/100mL 500mg-1000mg (5-10mL of a 10% solution) IV bolus for hyperkalemia
Diltiazem 0.25mg/kg) IV push over 2 minutes for atrial fibrillation or atrial flutter
Furosemide 0.5-1mg/kg IV over 1-2 minutes
Glucagon 3-10 IV slow IV push over 3-5 minutes
Lidocaine First dose: 1-1.5mg/kg IV/IO Second dose 0.5-0.75mg/kg IV push every 5 minutes Max: 3mg/kg
Magnesium Sulfate 1-2g in 10mL IV push for Torsades w/o pulse
Narcan 0.04-0.4mg IV/IM every 2-3 minutes or until respiratory drive returns
Sodium Bicarb 1 mEq/kg IV Bolus
Verapamil First dose: 2.5-5mg IV bolus over 2 minutes Second dose: 5-10mg every 15-30 minutes Max dose of 30 mg
