Representative Drugs

Bethanechol Cholinergic: increases detrusor muscle tone and GI motility. Adverse Reactions: bronchospasm, hypotension, tachycardia, seizures
Atropine Anticholinergic: preanesthetic; treatment of bradycardia, biliary colic, intestinal hypertonicity & muscarinic agonist poisoning. Adverse Reactions: glaucoma, tachycardia, severe bradycardia, gastric outlet obstruction, urinary retention, heat stroke.
Neostigmine Cholinesterase Inhibitor: used in treatment of neuromuscular disorders such as Myasthenia Gravis Adverse Reactions: arrhythmias, bradycardia, AV block, cardiac arrest, syncope, severe hypotension, respiratory arrest/depression, bronchospasm, seizures.
Physostigmine Cholinesterase Inhibitor: Adverse Reactions: cholinergic crisis, bradycardia, AV block, arrhythmias, severe hypotension, syncope, cardiac arrest, bronchospasm, respiratory depression/arrest, pulmonary edema, paralysis, seizures, hallucinations
Echothiophate Cholinesterase Inhibitor: binds irreversibly to acetylcholine binding site on pseudocholinesterase; treatment of glaucoma and incr. IOP.
Pralidoxime Antidote to inhibition of cholinesterase in organophosphate toxicity (reactivates cholinesterase). Adverse Reactions: cholinergic sx exacerbation
Edrophonium Reversible cholinesterase inhibitor: anesthesia adjunct; neuromuscular disorders; given as challenge dose to differential diagnosis of myasthenic crisis vs. cholinergic crisis. Adverse Reactions: cholinergic rxn, arrhythmias, bradycardia, hypotension,
Atracurium Neuromuscular Blocker: competitive, non-depolarizing nm blocker. Antagonizes ACh receptors on motor endplate. Adverse Reactions: Respiratory depression, apnea, malignant hyperthermia, seizures, bronchospasm, hypotension, cardiac arrest, arrhythmias
