Logic Models

TermDefinition
Inputs Resources or infrastructure available to the program planners.
Activities Interventions designed to encourage behavior change.
Outputs The evidence that the activities are producing the desired effect.
Effects The results or outcomes of the planned activities.
Purpose The mission of the program.
Context The environment in which the program will be implemented.
Inputs Trained PHNs, existing structures and systems
Activities Screening programs, education, counseling
Outputs People being screened, educated, or counseled
Effects Weight loss, improved health, appropriate parenting skills
Purpose To reduce the rates of obesity in adolescents
Context Low cost housing, limited open spaces
