Interventions for clients w/thyroid and Parathyroid problems

Thyroid hormones increase metabolic rate, heart rate, blood flow periphery, metabolism of fat, appetite, LOSE WEIGHT, has hyperglycemia
Hyperthyroidism Excess TSH from anterior pituitary/ thyroid works hard and enlarges
Graves disease autoimmune disorder-antibodies bind to thyroid causing enlargement (goiter, exophthalmus,heat intolerance)
Hyperthyroidism causes Autoimmune reaction(Graves), pituitary tumor, thyroiditis, thyroid cancer, thyroid meds
Hyperthyroidism manifestations see page 12, block number 2 in notes
Thyroid Storm Life threatening, uncontrolled hyperthyroidism, triggered by major stressor, trauma, or infection
Symptoms fever, tachycardia, systolic htn, agitated, anxious, tremors, psychotic, seizures and coma
emergency treatment maintain airway, give antithyroid meds, inderal for heart, prednisone, cooling blanket, antipyretics
Lab findings thyroid antibodies elevated, TSH decreased, T4 & T3 elevated
HYPOthyroidism Decrease TH and metabolism= obesity
Causes congenital, surgery, radiation, antithyroid meds, iodine deficiency, certain meds(lithium)
Manifestations see page 14 third row second block
Types of hypothyroidism iodine deficiency, hashimoto's thyroiditis(autoimmune)
Myxedema coma(hypothroid crisis) life threatening, occurs in inadequately treated hypothroidism
Myxedema manifestations edema around eyes, hands and feet, HYPOnatremia, HYPOglycemia, lactic acidosis, HYPOthermia, decreased cardiac output, HYPOtension, respiratory failure, coma
Labs for hypothyroidism Decrease TH's-T3 and T4, Increase TSHfrom pituitary trying to mak ethyroid produce more TH
hypothyroidism treatment Medications- thyroid replacement(levothyroid, synthroid, cytomel, euthroid) AND surgery
