What is a Health Assessment? a systematic collection of data.
Why do Nurses do a Health Assess.? 1.We gather baseline data( could be adm. data)2.Make nursing diagnoses (help develop a plan of care for the patient)3. Evaluate outcomes of care (successful or not)4. Make clinical judgements(the best way to attack the problem)
In doing a H.A the nurse wants to use? Therapeutic communication skills
Before conducting your H.A., you might want to tell patients that? you are going to be writting to reassure them that that is normal and apart of the assessment.
What type of questions do you want to use while doing your H.A.? open-ended questions (this can be apart of Alertx3)
Components of a Health Assessment? Lifestlye, Social Data, health history, Biographic data, Cheif complaint, history of any present illness, family illness, past medical history, psychological data
the primary source of data comes from? the patient
What are some sources of secondary data? family, friends, health care team, medical records
It is best to use a secondary source of data when the client is? unconscious or confused
Health History: a client's level of wellness? genreral health, does the client catch things easily, never catch things, etc.
Health History: Changes in life patterns recently been under a circumstance in their life that may have an impact on their health, ex. just getting through a divorce.
Health History: Socio-cultural different cultures are more prone to certain diseases or things.
Health History: Spiritual some religious people have certain principles and the nurse needs to take these principles into consideration.
Health History: Emotional Reaction to Illness do the nurse suspect that this patient might be suicidal, depressed, etc.
Types Of Assessment: Inital/Comprehensive Assessment? broad and includes everything
Focused/problem oriented assessment? narrow assessment "priority assessment"
Emergency Assessment? life vs. death (crucial that it be done immediatly)
Time- Lapsed Assessment? an assessment done over a period of time
What is the Demographic Data refering to? determines other things that may be subsectibale to? Based on the environment.
What are some biographic data? (8) Name, Age, Religion, Martial status, occupation, race, emergency contact, support system
Values/Beliefs (3) Religious preferences, Blood Transfusions, and Dietary restrictions
Stressors: What do they do? And what does the nurse need to consider? stressors precipitate illness; the nurse needs to be thinking about ways that the stressors can be prevented if neccessary.
What is another stressor concern that the nurse needs to consider? the way the client copes with their stressors. Do they eat, harm themselves, etc.
The patient response to the reason why they are seeking medical care should be? in qoetes or paraphrases
Onset means? where it all started
Duration? how long it last
Location? where does it hurt
Quality? on a scale how bad does it hurt
HPI means History of Presesnt Illness
