| Questions to ask: | Any numbness? tingling? weakness? changes in memory? hx of LOC? |
| A/O X3 | person, place, time? how are they groomed? note facial expression and speaking ability. |
| recall steps | ask pt to repeat series of numbers (immediate). name 3 objects and ask patient to recall them in 2 minutes (recent). Ask pt birth date or where born (remote). |
| more questions: | What is 4 plus 5? (math) Define apple. Interpret a proverb. |
| C1 and C2 | verbalize what they are: olfactory-sense of smell & optic-visual acuity by Snellen chart |
| C3, C4, C6 | Test EOMs, pupillary reaction to light and accommodation. |
| C5 | Test facial sensation c cotton swab. |
| C7 | Have pt smile and frown. |
| C8 | Hearing test (whisper 5 feet from them and have them repeat the word you siad). |
| C9, C10 | note quality of voice, ability to swallow and cough. look for symmetrical rise of uvula. |
| C11 | have pt raise shoulder against your resistance |
| C12 | stick out tongue |
| Tongue blades | assess sharp/dull sensation |
| Tuning fork | assess vibration on joint (great toe) |
| DTR | Assess at biceps, knee, and Achilles region. What would you do if they were having trouble c reflexes? (distract) |
| Meningitis a worry? | assess nuchal rigidity |
