Health Assessment-Abdominal

QuestionAnswer
What to ask pt: Any pain? hx of abd surgery? problems c GERD, ulcers, N/V?Any change in B/B? prg? when was your last menstrual period?
Appearance How does the pt appear today? how are they sitting? any acute distress noted?
inspect abd: is skin color consistent? any obvious hernia or mass?
Note contour: is it flat, schaphoid, rounded, or protuberant?
Auscultate abd: Listen in all 4 spots, lower rt 1st, then clockwise. how long do you listen if you don't hear anything? how would you listen for bruits?
Before palpation: where is the liver? where is the gallbladder? spleen? should you feel them normally?
Palpate why do you palpate last? show light and deep palpation. show how you attempt to palpate liver and spleen.
appendicities if complaints, how would you assess for appendicitis? what tests for pain in RLQ? what are they called? demonstrate.
