Health Assessment-Cardiac

QuestionAnswer
What questions to ask: Any chest pain? sob? weight gain? taking any medications? any concerns today?
Appearance How does the pt appear today? How are they sitting? Any acute distress noted?
Inspect neck Any pulsations or jugular veinous distention? what position for JVD?
Inspect chest Any pulsations noted? can you see the PMI?
Palpate chest Any thrills? Lifts? Heaves?
Auscultate chest any murmur present? Where? if so, listen for bruit at carotid
More auscultation Aortic spot? how would you document? Pulmonic? Where's Erb's point? Tricuspid spot? Mitral area?
S1-S4 Where is S1 the loudest? where is S2 the loudest? Any S3 or S4 noted? If you heard a murmur, how would you grade it? What does III/VI mean? IV/VI? V/VI? VI/VI?
Pulses how would you assess apical pulse versus radial? pedal? posterior tibial pulse? if you couldn't find the pulse what would you do?
