Healing by primary intention? edges approximated no or minimal scarring i.e. Surgical Closure
Healing by secondary intention? Left open to close by repair/ loss of tissue/pressure ulcers/open burn areas. Longer healing increase risk of infection scarring
Healing by tertiary intention? Left open and close surgically at later date. Widely separated deep extensive drainage /tissue debris closed later.
Stage I Intact nonblanchable redness usually over bony prominence may feel warmer
Stage II Partial thickness loss epidermis/dermis abrasion, blister shallow crater edema
Stage III Full thickness tissue/ loss of subcutaneous tissue you will see fat
Stage IV Full thickness tissue necrosis damge to muscle. Undermining tunneling
Unstageable ESCHAR/Slough obscures wound
