Gentamicin/garamycin, neomycin, streptomycin, tobramycin/ tobrax, Amikin, amikacin ABX/ Aminoglycosides: Ototoxic, nephrotoxic, high risk for anaphylaxis, assess CN VIII, encourage fluids
Keflex/ Cephalexin, Ceclor/ cefaclor, Rocephin, Maxipime, -cef ABX Cephalosporins:R/O bleeding, bone marrow suppression, take w food, cross allergy w penicillins, encourage fluids, VIT K for thrombophlebitis
Ciprofloxacin / Cipro, Levofloxacin / Levequin, Norfloxacin / Noroxin ABX/ Fluroquinolones: Photosensitivity, achilles tendon rupture, can be potentiated w probenecid, 3L/day fluids

Heparin, Lovenox Anticoagulant: PTT: 1.5 - 2.5 x control. 20-45 seconds, never give IM, antidote Protamine sulfate within 30 mintues
Warfarin, Coumadin Anticoagulant: PT: 9.5 - 12 sec, INR 2.0-3.5, antidotes Vit K. Therapeutic 13-18. No ASA, NSAIDS, Garlic, ginger, ginko.

Mag sulfate Anticonvulsant: Assess DTR/ resp arrest, used in OB preeclampsia, assess knee DTR prior to every dose
