Mode of action | For direct thrombin inhibitors: inhibit both free and fibrin-bound thrombin, preventing fibrinogen converting to fibrin, prevent thrombus formation |
Route/dose | Oral 10 days after THR VTE+ after surgery:110mg after 1-4hr of the surgery, then 220mg once daily AF, acute VTE: paren anticoagulant for 5 days before dabigatran, adults: elder= 150:110 x2 daily |
