Raise the bed to a comfortable level & assess the patient for orientation. Ask their name, todays date, current US president. Ask if they know where they are.

Take note of the way they are speaking, if its normal.

Assess PERRLA. Check to make sure eyes have consenual response to light,

Have them follow with their eyes in the shape of an H to test extraocular movement.

Have the patient look at something far away and then something close.

Determine if the patient is experiencing any numbness or tingling in the extremeties.

Lower the bed and have the patient walk, if able, to assess gait.    
🗑
Raise the bed rail, call bell in reach, check the patient is comfortable, and wash hands.

Document the procedure
