Cyclophosphamide (Cytoxan)   S/E: hemorrhagic cystitis; may be given with Mesna

Cisplatin (Platinol)   S/E: Renal toxicity, ototoxicity and severe nause/vomiting. May be administered with Amifostine as a renal protector

Paraplatin (Carboplatin)   Given after taxanes to decrease effects of myelosuppression

Eloxatin (Oxaliplatin)   May require calcium and magnesium infusions to reduce neurotoxicity; Patients should avoid exposure to cold when receiving this med.

Fluorouracil (5 FU)   S/E: GI toxicities, skin changes. Use ice chips PO 10-15 minutes pre- and post- IV bolus to reduce mucositis.

Methotrexate (Folex)   S/E: hepatotoxicty. High doses of this are given with Leucovorin rescue.

Doxorubicin (Adriamycin)   Lifetime dose of 500 - 550 mg/m2. Streaking or facial flushing indicates drug being administered too quickly. Patient's cardiac ejection fraction should be checked prior to initiation.

Blenoxane (Bleomycin)   Lifetime dose = 400 units. Test dose is recommended. Patients should have pulmonary function test prior to initiation.

Paclitaxel (Taxol)   Must use non-PVC bag and an inline .22 micron filter. Given first when administered with any 'Platin'.

Docetaxel (Taxotere)   Administer drug using non-PVC bag; however NO filter is required.

Etoposide (VP16)   S/E: hypotension, especially when given too fast.  
🗑
Vincristine (Oncovin)   S/E: peripheral neuropathies and alopecia. Vesicant. Should never administer more than 2mg.  
🗑
Rituximab (Rituxan)   S/E: high risk for infusion related reactions. Infusion should be increased in increments of 50mg/hr.

Trastuzumab (Herceptin)   S/E: infusion reactions, pulmonary toxicity left ventricular cardiac dysfunction. Monoclonal antibody given for breast cancer patients

Bortezomib (Velcade)   S/E: peripheral neuropathy. Administered SQ or IV- must rotate sites.  
🗑
Erlotinib (Tarceva)   S/E: rash, diarrhea, GI bleeding/bowel perforation. Oral medication taken on empty stomach.

Imatinib mesylate (Gleevec)   S/E: fluid retention, GI bleeding/perforation. Oral medication taken with meal and full glass of water.

Hydroxyurea (Hydrea)   S/E: myelosuppression, nausea, vomiting, stomatitis, mucositis. Oral medication (capsule).

Darbepoetin (Aransesp)   Growth Colony-Stimulating Factor used for the diagnosis of anemia

Filgrastim (G-CSF, Neupogen)   Growth Colony-Stimulating Factor used for the diagnosis of neutropenia
