You are the RN giving assignments to your CNA's at a long term care facility. You are expecting a new client from a rehab unit in a short time. When and at what intervals would you direct the CNA to take the new client's vital signs?   If delegating vital signs to CNA I would tell them to take them as soon as the client arrives in my care then every 4-8 hours unless values vary from baseline

How would you explain what you mean by vital signs and why you need them taken?   Vital signs are the assessment of vital or critical physiological functions. the vital signs are temperature, pulse, respiration and blood pressure. These values indicate a persons state of health and functioning of body systems and organs

If there is an unusually large blood loss. What effect would you expect to have on the patients VS? Would this info alter how often you would take the VS? Why?   It will reduce vascular volume so BP will fall, pulse will increase, respiration's will increase. I would take his vitals as soon as he gets into my care and take them every 5-15 minutes because the vitals are not normal and they vary from his baseline
