Antifungal and Antivirals

Ampotericin B facts highly toxic, monitor renal function weekly, damage to kidney may cause hypokalemia, may need supplements, monitor blood levels, combo: potentiated w/flucytosine, shake and bake
Ampotericin B SE and adverse effects fever, chills, rigors, nausea, H/A, phlebitis, nephrotoxicity, kidney damage (minimize w/ 1L of saline on tx days); mild reactions reduced by premedicating w/benadryl and ASA or acetaminophen
Amatadine facts 1st generation; parkinson drug; used for prophylaxis and influenza A; High risk pt (health care workers & family in extensive contact with pt); TETRAGENIC
Amatadine SE CNS (dizziness, nervousness, insomnia, difficulty concentrating), CV (CHF, ortostatic hypotension)
Acyclovir facts Antiviral; Active only against members of the herpes virus family; IV for initial and severe, Initial-topical because it reduces duration or viral shedding
Acyclovir uses Zovirax, HSV, VZV, CMV, Genital herpes by type 2 HSV
Action of acyclovir Inhibits viral replication by suppressing synthesis of viral DNA, selectivity based on ability of certain viruses to activate the drug, resistance occurs especially in transplants and AIDS
Adverse effects with acyclovir N/V/D, HA, vertigo, IV phlebitis, nephrotoxicity
Flucytosin Almost always giben with amphotericin B
Interferons Reduces ALT levels; SE: flu-like symptoms, depression is a concern, suicide has occured
