albuterol Proventil- Classification:S & R isomer bronchodilator, 2.5-10mg; Q1-12 hours; Notes:10 mg MAX over 1 hour. 10-20mg tx high K+.
levalbuterol Xopenex- Classification: R isomer; 0.63 or 1.25mg (adult) 0.31mg (peds); Q6-8 hours Notes:(possibly Q4). Selective for Beta 2 receptor sites.
salmeterol Serevent- Classification:maintenance bronchodilator. Dosage: 1 puff Diskus; Q12 hours. Notes:(DO NOT USE AS RESCUE BRONCHODILATOR)
ipratropium bromide Atrovent- Classification:anticholinergic bronchodilator; Dosage: 0.5mg liquid, 2 puffs MDI. Frequency per RCP. Notes:Opens larger airways
tiotropium bromide Spiriva DPI- Classification:long acting anticholinergic. Dosage:(1) 18mcg capsule. Once daily (Q24hours). Notes: Pierce capsule once.
Budesonide Pulmicort- Classification: corticosteroid. Dosage:0.25 or 0.5mg; 1-4 puffs MDI-BID. Notes:Rinse mouth to prevent thrush.
ipratropium bromide + albuterol Combivent MDI, DuoNeb- Dosage:2 puffs MDI, frequency per RCP. Notes: Onset= 15min, peaks 1-2 hours. Preferred for vent use.
dexamethasone sodium phosphate Decadron- Classification:corticosteroid. Dosage:1-4mg w. bronchodilator PER bronchodilator. Notes: Rinse mouth to prevent thrush.
fluticasone Florvent MDI Classification: corticosteroid Dosage: 1-4 puffs MDI, BID Notes:Rinse mouth to prevent thursh
acetylcysteine Mucomyst- Classification: mucolytic Doasge 10%= 6-10cc's; 20% 3-5 cc's per bronchodilator frequency. Notes: DO NOT mix w. aerosolized antibiotics;use with bronchodilator.
xylocaine Lidocaine- Classification: local anesthetic Dosage: 2%=20 mg/ml; 4%=40 mg/ml, PRN or per bronchodilator Notes: Nebulize QS pre-bronch; used for spastic airways too
tobramycin TOBI- Classification: antibiotic Dosage: 300 mg, BID. Notes: pre-tx w/ bronchodilator, DO NOT mix w/ mucolytic.
fluticasone + salmeterol Advair DPI- Classification: corticosteroid w/ maintenance bronchodilator. Dosage: 1 puff of 50mcg Salmeterol + 100, 250, or 500mcg fluticasone, BID. Notes: do not use as rescue inhaler, rinse mouth to prevent thrush.
mometasone furoate Asmanex DPI- Classification: corticosteroid Dosage: 220mcg/puff, BID or daily. Notes: cap activating dosing mechanism
formoterol Foradil DPI- Classification: maintenance & beta adrenergic bronchodilator. Dosage: 12 mcg capsule, BID. Notes: squeeze both sides to peirce capsule, d/c if order w/ adrenergic bronchodilators.
