| oral anti-coagulant/coumadin | open wounds, neurosurgical procedures, low BP, hepatic/renal disease |
| anti-platelet/plavix | active bleeding/severe hepatic disorder |
| thrombolytic agent/altepase | active bleeding, AV malfunction, ICP, GI ulcers |
| statin therapy/lipitor | active liver disease, pregnancy |
| loop diuretic/lasix | anuria, hepatic coma, severe electrolyte depletion |
| potassium sparing diuretic/aldactone | pts with anuria, significant acute renal failure, BUN and serum creatine |
| thiazide diuretic/diruil | anuria, hypokalemia, severe hepatic or renal impairment |
| ACE inhibitor/Enalapril | pt history of angioedema, renal impairment, low sodium levels |
| beta blocker/atenolol | do not use in patients with low HR/BP, AV block, cardiogenic shock |
