Muscle Length Tests

(Thomas Test) Iliopsoas/ Rectus Femoris Starting position: Supine with the table edge at the popliteal crease of both LE's
(Thomas Test) Iliopsoas/ Rectus Femoris Test: Examiner flexes both hips so that the pelvis is ~10* posteriorly tilted (double knee to chest). Examiner lowers one thigh to the table while the examiner or patient holds the other hip and knee in flexion.
(Thomas Test) Iliopsoas/ Rectus Femoris Negative test result: The thigh is extended to the table and the knee remains at least 80* of flexion(LE remains in the sagital plane). The iliopsoas are of normal length.
(Thomas Test) Iliopsoas/ Rectus Femoris Positive Test Result: If the thigh is not extended to the table, repeat the test holding the knee in extension. If the hip extends so that the thigh is on the table, then there is tightness in the rectus femoris.
(Thomas Test) Iliopsoas/ Rectus Femoris Positive Test Result: If the hip does not extend to the table, then there is shortness in the iliopsoas and possibly the joint capsule and ligaments.
(Thomas Test) Iliopsoas/ Rectus Femoris Measure: Measure the degree of ipsilateral hip extension.
(Eli Test) Rectus Femoris Starting position: Prone with both feet off of the edge of the table and hips in neutral add/abd/rotation
(Eli Test) Rectus Femoris Test: Flex the knee until firm pressure is felt. The end ROM is immediately before the ipsalateral hip flexes.
(Eli Test) Rectus Femoris Negative test result: Knee flexion>90* without hip flexion
(Eli Test) Rectus Femoris Positive test result: If the hip flexes when the knee is flexed beyond 90*
(Eli Test) Rectus Femoris Measure: Knee flexion before hip begins to flex.
Hamstrings (proximal) Starting Position: Supine with hips and knees extended
Hamstrings (proximal) Test: Flex the hip by lifting the LE off of the table;keep the knee in full extension with pressure over the anterior thigh.As the hip flexes,the pelvis and low back should flatten against the examining table.End ROM when Knee flexes, pelvis tilts post,spineflx
Hamstrings (proximal) Measure: hip flexion at end ROM
Hamstrings (distal), (popliteal angle) Starting Position: supine withe hips and knees extended
Hamstrings (distal), (popliteal angle) Test: Flex the hip to 90* and while allowing the knee to flex. Extend the knee to the end of ROM (immediately before the hip begins to extend).
Hamstrings (distal), (popliteal angle) Measure: Knee extension with hip flexed to 90*. The angle is calculated as a negative angle from the verticle positon (or full knee extension).
Gastrocnemius Measure ankle dorsiflexion ROM with the knee extended.
