QuestionAnswer
Mechanism Block binding of angiotensin II to receptors. Prevent vasoconstriction and aldosterone release. AT1 receptor mediates above AT2 may have anti – proliferative and vasodilatory effects. No effect on bradykinin pathway.
Kinetics High first pass effect. Highly protein bound. Renal and hepatic elimination. Telmisartan almost exclusively hepatic.
Side effects Same as ACE inhibitors but without the cough.
Drug Interactions Telmisartan with other hepatically cleared drugs. Increased digoxin and warfarin levels. Potassium-sparing diuretics and potassium supplements.
Names Losartan (Cozaar)‏. Candesartan (Atacand).‏ Eprosartan (Teveten).‏ Irbesartan (Avapro).‏ Telmisartan (Micardis).‏ Valsartan (Diovan).‏ Eprosartan (Teveten).‏ Olmesartan (Benicar).‏ Also in combination with thiazides and other antihypertensives.
