You reconstitute a 1 g vial by adding 8.5 mL of sterile water, resulting in a final concentration of 100 mg/mL. The order is for 400 mg IM. How many mL will you administer? 4 mL
A physician orders vancomycin 750 mg IV. Vial: 1 g powder. Directions: Reconstitute with 20 mL sterile water for a final concentration of 50 mg/mL. How many mL will you administer? 15 mL
A physician orders phenytoin 150 mg IV. Stock: phenytoin 250 mg/5 mL. How many mL will you administer? 3 mL
A physician orders phenytoin 200 mg IV. Stock: phenytoin 50 mg/mL (after reconstitution). How many mL will you administer? 4 mL
