objective data *lab *diagnostic studies (x-rays, CT, ultrasound, etc) *physical assessment
nursing diagnosis *different from a medical diagnosis *based on the analysis of the assessment data *may be actual or potential (Ex: risk for falls)
planning *characterized by goal setting *pt-centered *clearly states the expected change *acceptable to both pt & nurse *realistic & measurable *shared w/other HCPs *realistic deadline *identifies components for eval
goal setting *expected outcome *goes with planning
implementation *nursing actions necessary to accomplish the goals *pt teaching & education is important during this phase
