What are the recommendations for use of amiodarone in pregnancy? Not recommended to use 3 months prior to conceiving and all through the pregnancy

What are the possible impacts on the fetus if amiodarone is used during pregnancy? Severe bradycardia and thyroid dysfunction
Is amiodarone safe to use in breastfeeding? No
List the indications for amiodarone Treat + prevent recurring arrhythmias including supra/ventricular and ventricular tachycardias (AF, A flutter, WPWS, nodal etc)
What is the oral adult loading dose for amiodarone? 200mg tds for 1 week reducing to 200mg bd for 1 week
What is the oral adult maintenance dose for amiodarone? 100-200mg once daily
Explain the MOA of amiodarone Reduced response to unwanted electrical activity by prolonging action potential duration and refractory period of all cardiac tissue. Result = directly reduces cardiac tissue excitability and prevents conduction of unwanted electrical signals.
Discuss the precautions and contraindications to amiodarone C/I: allergy to iodine, heart block, sick sinus syndrome, symptomatic bradycardia P/C: poor liver + lung function, elderly, uncontrolled thyroid function, poor vision Screening: LFT, eye tests, lung function, ECG, thyroid
What are the key side effects associated with amiodarone? Alopecia, corneal deposits, optic nephropathy, hypo/hperthyroidism, pulomary toxicity (SOB, dry cough), bradycardia, worsening of arrythmia, hepatotoxicity, photosensitivity
What is a patient being monitored for whilst using amiodarone? Every 6 months: ECG, chest X-ray, LFT, lung function test, eye test, thyroid test, electrolytes, worsening of arrhythmia, BP, HR. Commencement of any other medications (RX or OTC) Other comorbidities: diabetes, obesity, HTN, Heart disease etc
List some medications that amiodarone can interact with Simvastatin, digoxin, warfarin, dabigatran, metoprolol, phenytoin, rifampicin + many more!
What are some counselling points to discuss with someone starting on amiodarone for the first time? Avoid sun exposure (8) Avoid grapefruit juice (18) Advise all health professionals you are taking this medication Need for regular health check ups (ECG, LFT etc) Ask Dr/Pharm before starting any new medication
What are the referral points for amiodarone? SOB, dry cough, blurred vision/visual disturbances, weight loss, muscle weakness, worsening arrhythmia, signs and symptoms of hypo/hyperthyroidism
What is the onset of action for amiodarone? 2 days to 3 weeks due to slow and variable oral absorption
