American Society of Health-System Pharmacists (ASHP) An association of pharmacists, pharmacy students, and technicians practicing in hospitals and health care systems, including home health care; has a long history of advocating for patient safety.
Automated dispensing system (ADS) Electronic system used to dispense medications.
Institute for Healthcare Improvement (IHI) A nonprofit organization committed to the improvement of health care by promoting promising concepts through safety, efficiency, and other patient-centered goals.
Institute for Safe Medication Practices (ISMP) A nonprofit organization devoted entirely to promoting safe medication use and preventing medication errors; it gathers info on drug errors and suggests new, safer standards to avoid such errors.
Medication error Any preventable event that may cause or lead to inappropriate medication use or patient harm.
Medication error prevention Methods used by pharmacy, medicine, nursing, and other allied health professionals to prevent medication error.
MedMARx A national Internet-accessible database that hospitals and health care systems use to track adverse drug reactions and medication errors.
MedWatch A program established by the US Food and Drug Administration (FDA) for reporting drug and medical product safety alerts and label changes; program also provides a voluntary adverse event reporting system for meds, medical products, and devices.
National Academy of Medicine (NAM) OR Institute of Medicine (IOM) Previously known as the IOM. Nonprofit organization provides scientifically informed analysis and guidance regarding health and health policy; projects include studies of drug safety systems in the US and recommendations for patient safety.
National Coordinating Council for Medication Error Reporting and Prevention (NCC-MERP) Founded by the USP, this is an independent council of more than 25 organizations gathered to address interdisciplinary causes of medication errors and strategies for prevention.
Pharmacy Technician Certification Board (PTCB) An organization that offers national certification for pharmacy techs in the US.
Risk Evaluation and Mitigation Strategy (REMS) A strategy for managing a known or potential serious risk associated with a drug or biological product.
Society for the Education of Pharmacy Technicians (SEPhT) A national pharmacy tech organization that promotes the education and training of pharmacy technicians; it provides links to med safety and quality practices for technicians.
United States Pharmacopeial (USP) Convention An independent organization that strives to ensure the quality, safety, and benefit of medicines and dietary supplements by setting standards and certification processes.
Examples of Drug-Drug interactions ACE Inhibitors --> Spironolactone= Increased serum potassium levels Ciprofloxacin--> Multivitamin with minerals= Decrease antibiotic absorption Digotoxin--> Verapamil & amiodarone= Digotoxin toxicity Theophylline--> Quinolones= Theophylline toxicity
Examples of Warfarin Interactions: Drug-Drug Aspirin: Possible increased risk for bleeding Phenytoin: increased levels of both drugs taken Quinolones, Sulfa drugs, Cimetidine, Heparin, Amiodarone, NSAIDS: Increased chance of bleeding
Examples of Warfarin Interactions: Supplements Gingko biloba: Increased chance of bleeding Vitamin K: Decreased activity of warfarin Garlic: Increased chance of bleeding Ginseng: Decreased activity of warfarin St. Johns wort: Decreased activity of warfarin
Examples of Warfarin Interactions: Foods Broccoli & other green veggies or foods high in Vitamin K: Decreased effect of warfarin Soybean and canola oils: Altered effect of warfarin Cranberry juice: Altered effect of warfarin
