TermDefinition
Advance Directive A binding legal document prepared and signed by a competent individual outlining the person's wishes should the person become incapacitated; for example, a medical power of attorney and a living will
Caregiver A person responsible for providing physical care and emotional support , usually in a home-care setting, to a person who is ill, disabled, or dependent
Host A server that provides data transfer, storage space, and other services to users at remote locations;a host has aunique domain name and is, in effect, the point at which a website originates
Living Will The part of an advanced directive that specifies which life-sustaining treatments (ex. Mechanical ventilation and tube feeding)should be administered or withheld if the person becomes incapacitated
Medical Power of Attorney (AKA "Durable power of attorney or health care proxy) The part of an advanced directive naming a trusted person to make medical decisions on the patient's behalf should they become unable to make such decisions independently
Online Community A virtual meeting space where like-minded people with common interests or concerns interact by using real-time chat rooms, asynchronous threaded discussions, discussion groups, social media, news groups, web confrenceing, and other tech
Patient Controlled Health Record The portion of a patient portal that contains data loaded by the patient and to which they alone may grant or deny access
Patient Portal A website that serves as an information transfer hub between patient and physician and that provides information and services, such as secure email,search capabilities, access to an online appointment book, and limited access to patient records
Personal Health Record (PHR) A secure, comprehensive record of health information that is controlled by the individual , creating a confidential electronic or paper-based file that is easy to access, manage, and share
Populate To complete a template or create a record by filling ina set of predetermined fields with information ranging from demographic data to values and measurements (for examplke, cital signs) to entire documents (such as correspondence and operative reports)
Social Networking The practice of using online communities to expand one's contacts and to exchange content, such as images and instant messages(also refers to: the phenomenon of this practice, which has created virtual communities with millions of members)
Wi-Fi A means of connecting wirelessly to the internet using local area network or router
